APPLICATION DEADLINE: NOVEMBER 20, 2024
Submission of this application form does not constitute acceptance into the tournament. You will be contacted by a Tournament Division Coordinator in regards to your team's acceptance into the Tournament on or before: December 1, 2024
TOURNAMENT DATES: January, 9 - 12, 2025
AAA TEAM ENTRY FEE: $2050.00 (4 Games Guaranteed)
NON-AAA TEAM ENTRY FEE: $ 1300.00 (3 Games Guaranteed)
REFUND POLICY: Any team accepted by the Tournament Committee; withdrawing after December 1, 2024 will forfeit their entry fee.

TOURNAMENT REGISTRATION REQUIREMENTS
The below requirements need to be fulfilled before your team will be officially accepted into the tournament:
- Application Form: Fill out the online application form with ALL necessary information
- Team Entry Fee: See payment options listed below. Required when registering you team.
ADDITIONAL INFORMATION REQUIRED BEFORE TEAMS ARE OFFICIALLY ACCEPTED
The information below is not required to register your team but needs to be received by November 15, 2024, for your team to be officially accepted into the tournament.
- Official Team Roster: Can be uploaded into the online application form when registering your team. The official team roster will be used to verify your team/players are registered with your Association
- Travel Permit: Can be uploaded into the online application form when registering your team
Payment is to be made by e-Transfer or by Cheque/Money Order and included with the completed package
Choose a payment method:
- Electronic Transfer (e-Transfer) - Preferred method of payment
- Contact our Tournament Treasurer Lynda Gall treasurer@liftlockatomhockey.ca to receive instructions on what to enter as the password for the e-Transfer
- Login to your online banking software and create a new e-Transfer
- Enter your team's Manager name and Head Coach name in the message section.
- Enter the text "Team Entry Fee" along with your Team Name (i.e. Peterborough Petes) as the security question
- Enter the password supplied by our Tournament Treasurer Lynda Gall
- Send e-Transfer to: treasurer@liftlockatomhockey.ca
- Cheque or Money Order
- Payable to: Peterborough Liftlock Atom Hockey Tournament
- Mail to: Lynda Gall, 150 DeClair Road Madoc, On K0K 2K0
- Non-Sufficient Fund (NSF) Cheques – A bank fee or surcharge of $20.00 in addition to entry fee.
- Cheque or Money Order must be received before your team is accepted into the tournament.
